Brazil's risk map is driven by the Amazon, and most coastal itineraries look very different from a jungle trip. Rio de Janeiro, São Paulo, Salvador, Florianópolis and the main beach destinations are malaria-free, while the Amazon basin — Amazonas, Acre, Rondônia, Roraima, Amapá and parts of Pará and Mato Grosso — carries year-round transmission where antimalarial tablets are normally recommended. Yellow fever is the defining Brazil issue: vaccination is recommended for almost all of the country including Rio, São Paulo state, Minas Gerais, Iguaçu Falls and the Pantanal after the outbreaks that spread into the southeast, it must be given at least ten days before arrival, and a certificate is required for onward travel to many neighbouring countries and often for returning travellers entering other South American states. Dengue is endemic nationwide with major seasonal epidemics from roughly January to May, and chikungunya, Zika and Oropouche circulate in the same regions, which makes bite prevention important for pregnant travellers in particular. Hepatitis A and typhoid are recommended for most trips, and tap water should be treated as unsafe outside good hotels in much of the country. Rabies pre-exposure vaccination is worth discussing for Amazon travel, caving and long rural stays, where bat exposure is a recognised risk and immunoglobulin is unavailable. Schistosomiasis is present in freshwater in the northeast and Minas Gerais, so avoid swimming in lakes and slow rivers, and leptospirosis follows urban flooding. Private hospitals in São Paulo and Rio are world class, but Amazon lodges and Pantanal ranches are many hours from care, so evacuation cover is the sensible baseline.
