Vietnam's risk profile changes sharply between the cities and the countryside. Hanoi, Ho Chi Minh City, Da Nang and Hoi An carry essentially no malaria risk, and CDC does not advise antimalarial tablets for travellers staying in urban areas or the main coastal tourist corridor. Malaria transmission persists in rural and forested zones, particularly the Central Highlands provinces of Dak Lak, Dak Nong, Gia Lai and Kon Tum, along the Cambodian and Laotian borders, and in parts of Binh Phuoc, where tablets are usually recommended for overnight rural stays. Dengue is the bigger everyday threat and circulates nationwide year-round, peaking in the southern wet season from roughly June to November, so repellent, long sleeves at dusk and dawn, and air-conditioned or screened rooms matter more than any pill. Hepatitis A and typhoid are standard additions for anyone eating street food or travelling outside major hotels, and Japanese encephalitis vaccination is worth discussing for stays over a month, rural homestays, or Mekong Delta and Red River Delta rice-farming areas during the May-to-October transmission season. Rabies is endemic and post-exposure immunoglobulin can be difficult to obtain outside Hanoi and Ho Chi Minh City, so pre-exposure vaccination is sensible for cyclists, motorbike travellers and long-stay visitors. International-standard care is available at FV Hospital and Vinmec in Ho Chi Minh City and Vinmec and Family Medical Practice in Hanoi, but Ha Giang, Sapa and the Central Highlands are many hours from that level of care, which is why evacuation cover is standard advice.
