Morocco is a low-risk destination with no malaria transmission, so antimalarial tablets are not recommended for Marrakech, Fes, Chefchaouen, Essaouira, the Sahara tours or the Atlas Mountains. No yellow fever certificate is required unless you are arriving from a country with transmission risk, which mainly affects travellers routing through sub-Saharan Africa. The realistic risks are foodborne and waterborne, so hepatitis A and typhoid are the two vaccines most travellers add; tap water is best avoided outside major hotels, and street food, fresh juices with ice, and undercooked meat in the souks are the usual causes of illness. Rabies is endemic and Morocco records human cases every year, with stray dogs and cats common in medinas and rural villages and post-exposure immunoglobulin difficult to source outside Rabat and Casablanca, so pre-exposure vaccination is a genuine consideration for cyclists, trekkers, children and long-stay travellers. The Atlas Mountains bring altitude into play: Toubkal reaches 4,167 metres and the standard two-day ascent is fast enough to cause acute mountain sickness in unacclimatised trekkers, while desert itineraries to Merzouga and the Draa Valley bring extreme heat, dehydration and sun exposure from late spring to early autumn. Routine boosters including tetanus, diphtheria and MMR should be current, and hepatitis B is worth considering for long stays or anyone who might need medical care. Casablanca and Rabat have good private clinics, but the High Atlas villages and deep Sahara are many hours from a hospital, so evacuation cover is worth having for trekking and desert trips.
