Japan is one of the lowest-risk destinations in Asia, and for most trips the honest answer is that routine vaccines are all that is needed. There is no malaria anywhere in Japan, no yellow fever requirement, and tap water is safe to drink across the country including Tokyo, Kyoto and Osaka. The one genuinely Japan-specific consideration is Japanese encephalitis, a mosquito-borne infection linked to rice paddies and pig farming; risk to typical city travellers is negligible, but CDC suggests vaccination for extended stays, repeated visits, or rural itineraries in western Honshu, Shikoku, Kyushu and Okinawa during the summer and early autumn transmission season. Measles, mumps and rubella immunity is worth confirming before travel, as Japan has recorded periodic measles clusters linked to imported cases in crowded urban settings. Hepatitis A and B are recommended only for longer stays, adventure travel or anyone likely to need medical or dental care, and rabies has been absent from Japan for decades so pre-exposure vaccination is not required. The practical hazards are seasonal rather than infectious: severe summer heat and humidity in Tokyo and Kyoto, typhoon season from roughly August to October, and altitude and rapid weather change if you are climbing Mount Fuji or trekking the Japanese Alps. Medical care is excellent but expensive without insurance, English-speaking clinics are concentrated in the big cities, and pharmacies are strict about imported medication, so bring prescriptions in original packaging with a doctor's letter.
